A aprendizagem baseada em projeto orientada pelos fundamentos da educação interprofissional é um modelo que pode contribuir para a formação de relacionamentos interpessoais, criatividade, empatia e colaboração na educação médica, por meio de uma colaboração mútua com profissionais de saúde da rede. Muito se fala da efetividade desse método no campo do ensino e aprendizagem médica, mas há a necessidade de incluir a importância do desenvolvimento de habilidades interprofissionais, com equipes colaborativas, em ações extensionistas, diante das necessidades locais no contexto da atenção primária, pensando na melhoria dos resultados de saúde. O objetivo deste trabalho é apresentar um relato de experiência de aprendizagem baseada em projeto de estudantes de Medicina no contexto da Estratégia Saúde da Família. Participaram deste trabalho estudantes do Módulo Integração Ensino, Serviço e Comunidade da Faculdade de Medicina da Universidade Federal dos Vales do Jequitinhonha e Mucuri que executaram, em colaboração com uma equipe interprofissional o projeto sobre a saúde do homem. Como resultado da análise qualitativa do feedback entre os integrantes, observaram-se mudanças no comportamento dos estudantes, com melhorias na comunicação, empatia e nas relações interpessoais, por meio do trabalho colaborativo com a equipe interprofissional. Esta experiência poderá ser adaptada para implementar o ensino e aprendizagem no projeto pedagógico orientado pela educação interprofissional na atenção primária.


Project-based learning guided by the fundamentals of interprofessional education is a model that can contribute to the formation of interpersonal relationships, creativity, empathy and collaboration within medical education, through mutual collaboration with health professionals in the health network. Much has been said about the effectiveness of this method in medical teaching and learning, but there is a need to include the importance of developing interprofessional skills, with collaborative teams, within extension actions, in view of local needs in the context of primary care, thinking about the improved health outcomes. The objective of this work was to present a report of a project-based learning experience of medical students in Family Health Strategy. Students from the Teaching, Service and Community Integration Module of the Faculty of Medicine of Universidade Federal dos Vales do Jequitinhonha e Mucuri participated in this work, executing in collaboration with an interprofessional team a project about men's health. As a result of the qualitative analysis of the feedback among the members, changes in student behavior were observed with improvements in communication, empathy and interpersonal relationships through collaborative work with the interprofessional team. This experience can be adapted to implement teaching and learning in the pedagogical project guided by interprofessional education in primary care.


El aprendizaje basado en proyectos y guiado por los fundamentos de la educación interprofesional es un modelo que puede contribuir a la formación de relaciones interpersonales, creatividad, empatía y colaboración dentro de la educación médica, a través de la colaboración mutua con los profesionales de la salud en la red de salud. Mucho se habla de la efectividad de este método dentro de la enseñanza y el aprendizaje médico, pero es necesario incluir la importancia del desarrollo de habilidades interprofesionales, con equipos colaborativos, dentro de las acciones de extensión, frente a las necesidades locales en el contexto de la atención primaria, pensando sobre los mejores resultados de salud. El objetivo de este trabajo es presentar un informe de experiencia de aprendizaje basado en proyectos de estudiantes de medicina en la Estrategia de Salud Familiar. Participaron en este trabajo estudiantes del Módulo Integración Enseñanza, Servicio y Comunidad de la Facultad de Medicina de la Universidade Federal dos Vales do Jequitinhonha e Mucuri que ejecutaron en colaboración con un equipo interprofesional el proyecto sobre la salud del hombre. Como resultado del análisis cualitativo de la retroalimentación entre los integrantes, se observaron cambios en el comportamiento de los estudiantes con mejoras en la comunicación, la empatía y las relaciones interpersonales a través del trabajo colaborativo con el equipo interprofesional. Esta experiencia puede adaptarse para implementar la enseñanza y el aprendizaje en el proyecto pedagógico guiado por la educación interprofesional en atención primaria.

Resumo Introdução: A Cultura de Segurança do Paciente é considerada um importante componente estrutural dos serviços, que favorece a implantação de práticas seguras e a diminuição da ocorrência de eventos adversos. Objetivo: Identificar os fatores associados à cultura de segurança do paciente nas unidades de terapia intensiva adulto em hospitais de grande porte da região Sudeste do Brasil. Método: Estudo transversal do tipo survey e multicêntrico. Participaram 168 profissionais de saúde de quatro unidades (A, B, C e D) de terapia intensiva adulto. Foi utilizado o questionário "Hospital Survey on Patient Safety Culture". Considerou-se como variável dependente o nível de cultura de segurança do paciente e variáveis independentes aspectos sociodemográficos e laborais. Foram usadas estatísticas descritivas e para a análise dos fatores associados foi elaborado um modelo de regressão logística múltipla. Resultados: Identificou-se associação entre tipo de hospital com onze dimensões da cultura de segurança, quanto à função a categoria profissional médico, técnico de enfermagem e enfermeiro foram relacionadas com três dimensões; o gênero com duas dimensões e tempo de atuação no setor com uma dimensão. Conclusão: Evidenciou-se que o tipo de hospital, categoria profissional, tempo de atuação no setor e gênero foram associados às dimensões de cultura de segurança do paciente.


Resumen Introducción: La cultura de seguridad del paciente se considera un componente estructural importante de los servicios, que favorece la aplicación de prácticas seguras y la reducción de la aparición de acontecimientos adversos. Objetivo: Identificar los factores asociados a la cultura de seguridad del paciente en unidades de terapia intensiva adulto en hospitales de la región Sudeste del Brasil. Metodología: Estudio transversal de tipo encuesta y multicéntrico. Participaron 168 profesionales de salud de cuatro unidades (A, B, C y D) de terapia intensiva adulto. Se utilizó el cuestionario "Hospital Survey on Patient Safety Culture". Se consideró como variable dependiente el nivel de cultura de seguridad del paciente y variables independientes los aspectos sociodemográficos y laborales. Fueron usadas estadísticas descriptivas y, para analizar los factores asociados, fue elaborado un modelo de regresión logística múltiple. Resultados: Se identificó asociación entre tipo de hospital con once dimensiones de cultura de seguridad del paciente. En relación a la función, personal médico, técnicos de enfermería y personal de enfermería fueron asociados con tres dimensiones, el género con dos dimensiones y tiempo de actuación con una dimensión en el modelo de regresión. Conclusión: Se evidenció que el tipo de hospital, función, tiempo de actuación en el sector y género fueron asociados a las dimensiones de la cultura de seguridad del paciente.


Abstract Introduction: Patient safety culture is considered an important structural component of the services, which promotes the implementation of safe practices and the reduction of adverse events. Objective: To identify the factors associated with patient safety culture in adult intensive care units in large hospitals in Belo Horizonte. Method: Cross-sectional survey and multicenter study. A total of 168 health professionals from four units (A, B, C and D) of adult intensive care participated. The questionnaire "Hospital Survey on Patient Safety Culture" was used. The patient's level of safety culture was considered as a dependent variable, and sociodemographic and labor aspects were the independent variables. Descriptive statistics were used and a multiple logistic regression model was developed to analyze the associated factors. Results: An association was identified between the type of hospital and eleven dimensions of the safety culture. In terms of function, the doctors, nursing technicians, and nurse were related to three dimensions; gender with two dimensions, and time working in the sector with one dimension. Conclusion: It was evidenced that the type of hospital, function, time working in the sector, and gender were associated with the dimensions of patient safety culture.

RESUMO

Fragility fractures are associated with high morbidity and mortality. An interdisciplinary collaboration and an individualized, patient-centered approach are essential to ensure an optimized preoperative period and to improve perioperative safety. Preoperative responsibilities of trauma surgery include in the first step the identification of fragility fractures and the necessity for geriatric involvement. Orthogeriatric co-management (OCM) focuses on the identification of the medical, functional and social needs of the patient. In the preoperative period attention is focussed on acute diseases in need of treatment that have a negative impact on the course of further treatment and the prevention of delirium.

Palliative care in the field of urology has largely been limited to adult oncologic conditions. Although there is a plethora of established literature suggesting the advantageous impact of palliative care, there is limited integration of palliative care in adult urology. This underutilization is further exacerbated in pediatric urology, and palliative care in pediatric urology remains an underexplored area despite the prevalence of several life-limiting conditions in this patient population. This paper highlights the potential need for palliative care intervention in a variety of urologic conditions in the pediatric population, including congenital lower urinary tract obstruction, neurogenic bladder dysfunction, exstrophy-epispadias complex, and congenital bilateral renal agenesis. Each condition poses unique challenges that can be addressed with the inclusion of a palliative care team, including decision-making spanning prenatal-neonatal-pediatric periods, acute and chronic symptom management, family relations, body image issues, risk of recurrent hospitalizations and surgeries, and potentially fatal complications. Alongside standard urologic interventions, palliative care can serve as an additional means of addressing physical and psychosocial symptoms experienced by pediatric urology patients to enhance the quality of life of patients and their families.

Neurocutaneous syndromes comprise a heterogeneous group of congenital or hereditary conditions that are known to be associated with the risk of different disorders and complications. Two of the most common neurocutaneous syndromes are Neurofibromatosis type 1 (NF1) and Tuberous Sclerosis Complex (TSC). Although there appears to be a general consensus on the importance of a multidisciplinary approach in managing these cases, there is still very little emphasis in discussions addressed in the literature on the role of dentistry in accordance with the perspective of comprehensive care. Evidence-based propositions, together with a broad discussion of new insights in this regard, should have the ability to strongly impact related future perspectives, aiming for greater advances and better outcomes for these patients. In this review article, the authors discuss updated general aspects of NF1 and TSC, and the potential additional roles of dentistry, in addition to addressing suggestions for actions in dentistry at related levels of care, as well as priorities for future research.

OBJECTIVE: To provide evidence-based recommendations for the management of chronic pelvic pain in females. TARGET POPULATION: This guideline is specific to pelvic pain in adolescent and adult females and excluded literature that looked at pelvic pain in males. It also did not address genital pain. BENEFITS, HARMS, AND COSTS: The intent is to benefit patients with chronic pelvic pain by providing an evidence-based approach to management. Access to certain interventions such as physiotherapy and psychological treatments, and to interdisciplinary care overall, may be limited by costs and service availability. EVIDENCE: Medline and the Cochrane Database from 1990 to 2020 were searched for articles in English on subjects related to chronic pelvic pain, including diagnosis, overlapping pain conditions, central sensitization, management, medications, surgery, physiotherapy, psychological therapies, alternative and complementary therapies, and multidisciplinary and interdisciplinary care. The committee reviewed the literature and available data and used a consensus approach to develop recommendations. Only articles in English and pertaining to female subjects were included. VALIDATION METHODS: The authors rated the quality of evidence and strength of recommendations using the Grading of Recommendations Assessment, Development and Evaluation (GRADE) approach. See online Appendix A (Tables A1 for definitions and A2 for interpretations of strong and conditional [weak] recommendations). INTENDED AUDIENCE: Family physicians, gynaecologists, urologists, pain specialists, physiotherapists, and mental health professionals. TWEETABLE ABSTRACT: Management of chronic pelvic pain should consider multifactorial contributors, including underlying central sensitization/nociplastic pain, and employ an interdisciplinary biopsychosocial approach that includes pain education, physiotherapy, and psychological & medical treatments. SUMMARY STATEMENTS: RECOMMENDATIONS.

RESUMO

Background: Pancreatic ductal adenocarcinoma (PDAC) is a common and lethal cancer. From diagnosis to disease staging, response to neoadjuvant therapy assessment and patient surveillance after resection, imaging plays a central role, guiding the multidisciplinary team in decision-planning. Review aims and findings: This review discusses the most up-to-date imaging recommendations, typical and atypical findings, and issues related to each step of patient management. Example cases for each relevant condition are presented, and a structured report for disease staging is suggested. Conclusion: Despite current issues in PDAC imaging at different stages of patient management, the radiologist is essential in the multidisciplinary team, as the conveyor of relevant imaging findings crucial for patient care.

Comorbidities may contribute to inadequate response to therapy and accelerate disability in various rheumatic diseases such as rheumatoid arthritis (RA), systemic lupus erythematosus (SLE), and psoriatic arthritis (PsA). Cardiovascular, oncological, and infectious comorbidities are common in rheumatic patients. The rehabilitation of patients with inflammatory rheumatic diseases (IRDs) with comorbidities requires a multidisciplinary approach to improving patients' functional mobility, slowing down the disease progression and minimizing the risks of complications. The evidence suggests that cardiac rehabilitation can be implemented in daily practice in patients with IRDs to reduce mortality for those with established risk factors. Physical exercises reduce the severity, improve the clinical course, and reduce hospitalization rates in patients with rheumatic diseases. A rehabilitation program with focused physical therapy can lead to functional improvements and reduction of disease activity in patients with lowered quality of life (QoL). Health professionals should provide evidence-based recommendations for patients with rheumatic diseases and comorbidities to initiate the self-management of their diseases and prevent complications.

PURPOSE: We conducted a study to understand primary care physician (PCP) and urologist perspectives on determinants of active surveillance care delivery for men with low-risk prostate cancer. METHODS: We conducted in-depth, semistructured, virtual interviews with a purposive sample of 19 PCPs and 15 urologists between June 2020 and March 2021. We used the behavioral theory-informed Theoretical Domains Framework to understand barriers to and facilitators of active surveillance care delivery. Interviews were recorded, transcribed, and deductively coded into framework domains and constructs by 3 independent coders. Participant recruitment continued until data saturation by group. RESULTS: Our study included 19 PCPs (9 female; 4 in community practices, 15 in academic medical centers) and 15 urologists (3 female; 5 in private practice, 3 in academic medical centers). The most commonly reported Theoretical Domains Framework domains affecting active surveillance care were (1) knowledge and (2) environmental context and resources. Although urologists were knowledgeable about active surveillance, PCPs mentioned limitations in their understanding of active surveillance (eg, what follow-up entails). Both groups noted the importance of an informed patient, especially how a patient's understanding of active surveillance facilitates their receipt of recommended follow-up. Physicians viewed patient loss to follow-up as a barrier, but identified a favorable organizational culture/climate (eg, good communication between physicians) as a facilitator. CONCLUSIONS: With patients increasingly involving their PCPs in their cancer care, our study presents factors both PCPs and urologists perceive (or identify) as affecting optimal active surveillance care delivery. We provide insights that can help inform multilevel supportive interventions for patients, physicians, and organizations to ensure the success of active surveillance as a management strategy for low-risk prostate cancer.

INTRODUCTION: Abdominal wall reconstruction (AWR) is an emerging subspecialty within general surgery. The practice of multidisciplinary team (MDT) meetings to aid decision making and improve patient care has been demonstrated, with widespread acceptance. This study presents our initial experience of over 150 cases of complex hernia patients discussed in a newly established MDT setting. METHODS: From February 2020 to July 2022 (30-month period), abdominal wall MDTs were held bimonthly. Key stakeholders included upper and lower gastrointestinal surgeons, a gastrointestinal specialist radiologist, a plastic surgeon, a high-risk anaesthetist and two junior doctors integrated into the AWR clinical team. Meetings were held online, where patient history, past medical and surgical history, hernia characteristics and up-to-date computed tomography scans were discussed. RESULTS: Some 156 patients were discussed over 18 meetings within the above period. Ninety-five (61%) patients were recommended for surgery, and 61 (39%) patients were recommended for conservative management or referred elsewhere. Seventy-eight (82%) patients were directly waitlisted, whereas seventeen (18%) required preoperative optimisation: three (18%) for smoking cessation, eleven (65%) for weight-loss management and three (18%) for specialist diabetic assessment and management. In total, 92 (59%) patients (including operative and nonoperative management) have been discharged to primary care. DISCUSSION: A multidisciplinary forum for complex abdominal wall patients is a safe process that facilitates decision making, promotes education and improves patient care. As the AWR subspecialty evolves, our view is that the "complex hernia MDT" will become commonplace. We present our experience and share advice for others planning to establish an AWR centre.

AIMS: To identify postoperative interventions and quality improvement initiatives used to prevent wound complications in patients undergoing colorectal surgeries, the types of activities nurses undertake in these interventions/initiatives and how these activities align with nurses' scope of practice. DESIGN: A scoping review. DATA SOURCES: Three health databases were searched, and backward and forward citation searching occurred in April 2022. Research and quality improvement initiatives included focussed on adult patients undergoing colorectal surgery, from 2010 onwards. Data were extracted about study characteristics, nursing activities and outcomes. The 'Dimensions of the scope of nursing practice' framework was used to classify nursing activities and then the Patterns, Advances, Gaps, Evidence for practice and Research recommendations framework was used to synthesise the review findings. RESULTS: Thirty-seven studies were included. These studies often reported negative wound pressure therapy and surgical site infection bundle interventions/initiatives. Nurses' scope of practice was most frequently 'Technical procedure and delegated medical care' meaning nurses frequently acted under doctors' orders, with the most common delegated activity being dressing removal. CONCLUSION: The full extent of possible interventions nurses could undertake independently in the postoperative period requires further exploration to improve wound outcomes and capitalise on nurses' professional role. IMPACT STATEMENT: Nurses' role in preventing postoperative wound complications is unclear, which may inhibit their ability to influence postoperative outcomes. In the postoperative period, nurses undertake technical activities, under doctors' orders to prevent wound infections. For practice, nurses need to upkeep and audit their technical skills. New avenues for researchers include exploration of independent activities for postoperative nurses and the outcomes of these activities. IMPLICATIONS FOR THE PROFESSION AND/OR PATIENT CARE: There may be opportunities to broaden nurses' scope of practice to act more autonomously to prevent wound complication. REPORTING METHOD: Scoping Reviews (PRISMA-ScR) checklist. PATIENT OR PUBLIC CONTRIBUTION: A health consumer interpreted the data and prepared the manuscript.

Objective: to investigate the relationship between team climate and job satisfaction among professionals working in mobile pre-hospital care. Method: this is a quantitative, correlational study carried out in a mobile pre-hospital care service in the São Paulo Metropolitan Region. The participants were 95 professionals, allocated to 40 teams, who answered three questionnaires: sociodemographic/labor data, Team Climate Scale and S20/23 Job Satisfaction Scale. Descriptive statistics and multilevel linear models were used for the analysis, including moderation effects. The Backward method was used to ascertain the order of significance. Results: in the models, the relationships between satisfaction with hierarchical relationships and the factor "support for new ideas" moderated for men and "task orientation" for women were significant. For satisfaction with the physical environment, "working hours" and "participation in the team" were significant and, for intrinsic satisfaction, the regime, working hours and the factors "team objectives", "participation in the team" and "support for new ideas" remained significant, as did the moderation effect between length of service, "participation in the team" and "support for new ideas". Conclusion: team climate is influenced by job satisfaction in a heterogeneous way and the moderating effect of this relationship is associated with gender and length of service.


Objetivo: examinar la relación entre el clima en equipo y la satisfacción laboral de los profesionales que trabajan en servicios de atención prehospitalaria móvil. Método: estudio cuantitativo y correlacional realizado en un servicio de atención prehospitalaria móvil en la Región Metropolitana de São Paulo. Participaron 95 profesionales, distribuidos en 40 equipos, que respondieron a tres cuestionarios: datos sociodemográficos/laborales, Escalas de Clima en Equipo y de Satisfacción Laboral S20/23. Para el análisis, se utilizaron estadísticas descriptivas y modelos lineales multiniveles, incluyendo efectos de moderación. Se empleó el método Backward para determinar el orden de significancia. Resultados: en los modelos, fueron significativas las relaciones entre la satisfacción con las relaciones jerárquicas y el factor "apoyo para nuevas ideas" moderado por el género masculino y "orientación hacia las tareas" en mujeres. Para la satisfacción con el ambiente físico, fueron significativos "jornada laboral" y "participación en el equipo", y para la satisfacción intrínseca, se mantuvieron significativos el régimen, la jornada y los factores "objetivos del equipo", "participación en el equipo" y "apoyo para ideas nuevas", y el efecto de moderación entre el tiempo de actuación, "participación en el equipo" y "apoyo para ideas nuevas". Conclusión: el clima en equipo es influenciado por la satisfacción laboral de manera heterogénea y el efecto moderador de esta relación se asocia con el género y el tiempo de actuación en el servicio.

Resultados: nos modelos, foram significantes as relações entre satisfação com relações hierárquicas e fator "apoio para novas ideias" moderado ao gênero homem e "orientação para as tarefas" às mulheres. Para satisfação com ambiente físico, foram significantes "jornada de trabalho" e "participação na equipe" e, para satisfação intrínseca, permaneceram significantes o regime, a jornada e os fatores "objetivos da equipe", "participação na equipe" e "apoio para ideias novas", e efeito de moderação entre tempo de atuação, "participação na equipe" e "apoio para ideias novas". Conclusão: clima em equipe é influenciado pela satisfação no trabalho de modo heterogêneo e o efeito moderador dessa relação associa-se ao gênero e ao tempo de atuação no serviço.

RESUMO Objetivo O objetivo deste estudo foi validar, com base nos processos de resposta, o Instrumento Multiprofissional de Rastreio para o Risco de Broncoaspiração em Ambiente Hospitalar, direcionado à população idosa. Método O instrumento foi aplicado por juízes em pacientes distintos e selecionados de forma aleatória. Após a aplicação, os juízes foram entrevistados para que fosse possível verificar a impressão deles quanto à relevância dos itens e quanto a interpretação sobre o conteúdo escrito, bem como a questões gramaticais e semânticas. Foram consideradas sugestões de acréscimo de alternativas de perguntas e de respostas, além de propostas de adequação de questões que compunham o instrumento. As reações não verbais, tais como expressões faciais que sugeriram dúvidas ou hesitações, por parte dos juízes, em relação ao instrumento, também foram analisadas. Resultados A concordância dos juízes em relação a cada item do dispositivo foi calculada pelo Índice de Validade de Conteúdo (IVC), e pelo Coeficiente de Correlação Intraclasse (CCI), sendo que seus resultados demonstraram alto nível de concordância. Através das sugestões dos juízes, elaborou-se uma nova versão do Instrumento Multiprofissional de Rastreio para o Risco de Broncoaspiração em Ambiente Hospitalar à população idosa. Conclusão Os resultados obtidos demonstraram que a validade do Instrumento Multiprofissional para Rastreio do Risco de Broncoaspiração em Ambiente Hospitalar junto à população idosa, baseada nos processos de respostas, foi alcançada.


ABSTRACT Purpose The objective of this study was to validate the Multiprofessional Screening Instrument for Broncho-aspiration Risk in Hospital Environment, which is aimed at the elderly population, based on response processes. Methods Judges applied the instrument to different patients and randomly selected. After the application, the judges were interviewed so that it was possible to verify their impression regarding the relevance of the items about their interpretation of the written content, as well as grammatical and semantic issues. Suggestions for adding alternative questions and answers were considered, as well as proposals for adapting the questions that made up the instrument. Non-verbal reactions, such as facial expressions that suggested doubts or hesitations, by the judges concerning the instrument were also analyzed. Results The agreement of the judges concerning each item of the device was calculated by the Content Validity Index (CVI) and by the Intraclass Correlation Coefficient (ICC), and their results showed a high level of agreement. Through the suggestions of the judges, a new version of the Multi-professional Screening Instrument for the Risk of Broncho-aspiration in a Hospital Environment in the Elderly was elaborated. Conclusion The results obtained showed that the validity of the Multi-professional Instrument for Screening the Risk of Broncho-aspiration in the Hospital Environment with the elderly population, based on the response processes, was achieved and makes it a promising device to assist professionals in hospital care for the elderly.

Objetivo: produzir um guia cuidativo-educacional para e com os profissionais de saúde de uma Unidade de Terapia Intensiva, a partir da compreensão de suas práticas vivenciadas no manejo do óbito neonatal. Método: estudo metodológico, qualitativo, guiado pelo Itinerário de Pesquisa de Paulo Freire, constituído por três etapas: Investigação temática, Codificação e Descodificação e Desvelamento crítico. Realizado em uma maternidade pública em Manaus, Amazonas, Brasil, com a participação de 24 profissionais. Resultados: o guia foi organizado em três capítulos sobre os cuidados e preparo do bebê; orientações para a equipe de saúde do que fazer diante da perda neonatal; apoio a equipe e direitos maternos diante da perda. Conclusão: a tecnologia foi produzida com uma estratégia metodológica que contribuiu para a compreensão das práticas de cuidado no manejo do óbito neonatal, marcadas por sentimentos traumáticos, que de forma coletiva, os participantes se sentirem estimulados a buscar intervenções sistematizadas, acolhedoras e humanizadas.


Objective: to produce a care-educational guide for and with health professionals in an Intensive Care Unit, based on an understanding of their practices in neonatal death management. Method: a qualitative and methodological study guided by Paulo Freire's Research Itinerary, consisting of three stages: Thematic investigation; Coding and Decoding; and Critical Unveiling. It was carried out at a public maternity hospital in Manaus, Amazonas, Brazil, with the participation of 24 professionals. Results: the guide was organized into three chapters on care and preparation of the neonate; guidelines for the health team on what to do in the face of neonatal loss; support for the team; and maternal rights facing the loss. Conclusion: the technology was produced with a methodological strategy that contributed to understanding care practices in neonatal death management, marked by traumatic feelings, which collectively encouraged the participants to seek systematized, welcoming and humanized interventions.


Objetivo: crear una guía educativa y de cuidados para y con los profesionales de la salud de una Unidad de Cuidado Intensivo, a partir d comprender las prácticas que experimentan en el manejo de la muerte neonatal. Método: estudio metodológico y cualitativo, guiado por el Itinerario de Investigación de Paulo Freire, constituido por tres etapas: Investigación temática, Codificación y Descodificación, y Desvelamiento crítico. El estudio se realizó en una maternidad pública de Manaus, Amazonas, Brasil, con participación de 24 profesionales. AIM: To reach a multidisciplinary consensus on managing patients with type 2 diabetes among specialists in family medicine, cardiology, endocrinology, internal medicine, and nephrology. METHODS: A two-round Delphi study was conducted using a questionnaire with 68 positive/negative statements distributed in four thematic blocks on diabetes management: early diagnosis and prediabetes, referral criteria, treatment and comorbidities, and clinical management. The expert panel was composed of 105 physicians from different specialties (family medicine, cardiology, endocrinology, internal medicine, and nephrology) with experience in managing patients with diabetes and who were members of a diabetes-related society. RESULTS: Response rates for the first and second rounds were 86.7 and 75.2%, respectively. After both rounds, a consensus was reached on 52 (76.5%) items. The recommendations with the highest degree of consensus (median = 10, IQR = 0.00) were related to anti-smoking education, cardiovascular risk factor target control, and diabetic kidney disease. There were significant differences between family physicians and other specialties for some items. CONCLUSIONS: This study provides a set of recommendations for diabetes management agreed upon by specialists from different healthcare settings.

Introduction: Collaboration between different health professionals is essential to ensure safe and quality prenatal care and favorable birth outcomes. Objective: To describe interprofessional prenatal care and management strategies in the context of primary health care. Materials and method: Qualitative research conducted between August and December 2022. Data was collected using an instrument to analyze the content of infographics produced by 26 professionals working in primary health care in southern Brazil, as an evaluation activity for a qualification course. The content of the infographics was analyzed using the coding process proposed by Minayo. Results: The data analyzed enabled three thematic categories to be identified: "The necessary intersectoral and interprofessional coordination;" "Responsibilities built and shared between pregnant women and professionals;" "Planning and qualification of workflows and processes." Conclusions: Interprofessional prenatal care and management strategies in the context of primary health care are associated with overcoming fragmented and dichotomous approaches, as well as the prospect of more participatory, dialogic, and interactive prenatal care and management processes, with the inclusion of various health actors.


Introducción: la colaboración entre diferentes profesionales del área de la salud es esencial para garantizar atención prenatal segura y de calidad, y resultados favorables en el parto. Objetivo: describir estrategias interprofesionales de la gestión y atención prenatal en el contexto de la atención primaria de salud. Materiales y método: investigación cualitativa realizada entre agosto y diciembre de 2022. Los datos se recogieron mediante un instrumento de análisis del contenido de infografías producidas por 26 profesionales que trabajan en la atención primaria de salud en el sur de Brasil, como actividad de evaluación de un curso de cualificación. El contenido de las infografías se analizó mediante el proceso de codificación propuesto por Minayo. Resultados: los datos analizados permitieron identificar tres categorías temáticas: "La necesaria coordinación intersectorial e interprofesional"; "Responsabilidades construidas y compartidas entre gestantes y profesionales"; "Planificación y cualificación de flujos y procesos de trabajo". Conclusiones: las estrategias interprofesionales de gestión y atención prenatal en el contexto de la atención primaria de salud se asocian a la superación de enfoques fragmentados y dicotómicos, así como a la perspectiva de procesos de gestión y atención prenatal más participativos, dialógicos e interactivos con la inclusión de diversos actores de la salud.

Objetivo: descrever estratégias interprofissionais de gestão e atenção pré-natais no contexto da atenção primária à saúde. Materiais e método: pesquisa qualitativa realizada entre agosto e dezembro de 2022. Os dados foram coletados a partir de um instrumento de análise do conteúdo de infográficos produzidos por 26 profissionais que atuam na atenção primaria à saúde, no sul do Brasil, como atividade avaliativa de um curso de qualificação. O conteúdo dos infográficos foi analisado com base no processo de codificação proposto por Minayo. Objetivo: compreender o processo de alta hospitalar de crianças dependentes de tecnologias por profissionais de unidades pediátricas e neonatais. Método: estudo descritivo com os profissionais da assistência à criança dependente de tecnologias de um Hospital Universitário entre julho e setembro de 2020. Os dados foram obtidos foram exportados para o software Iramuteq e interpretados à luz da teoria de análise de conteúdo. O estudo possui aprovação pelo Comitê de Ética e Pesquisa. Resultados: a nuvem de palavras contextualizou e forneceu subsídios para a identificação de duas categorias: Compreensão da necessidade de trabalho em equipe para planejamento de alta hospitalar e percepção da necessidade de implementação de ferramentas como protocolos e fluxos para o direcionamento do preparo para alta e continuidade do cuidado. Conclusão: os profissionais referem que a alta hospitalar é realizada conforme a rotina assistencial e ainda segue um modelo medicalocêntrico.


Objective: to understand the process of hospital discharge of technology-dependent children by professionals from pediatric and neonatal units. Method: descriptive study with technology-dependent child care professionals at a University Hospital between July and September 2020. The data obtained was exported to the Iramuteq software and interpreted in light of content analysis theory. The study is approved by the Research Ethics Committee. Results: the word cloud contextualized and provided support for the identification of two categories: Understanding the need for teamwork to plan hospital discharge and perception of the need to implement tools such as protocols and flows to guide preparation for discharge and continuity of care. Conclusion: professionals report that hospital discharge is carried out according to the care routine and still follows a medical-centric model.


Objetivo: comprender el proceso de alta hospitalaria de niños dependientes de tecnología por parte de profesionales de unidades de pediatría y neonatología. Método: estudio descriptivo con los profesionales de cuidado al niño que depende de tecnologías en un Hospital Universitario, entre julio y septiembre de 2020. Los datos obtenidos se exportaron al software Iramuteq y se interpretaron a la luz de la teoría del análisis de contenido. El estudio es aprobado por el Comité de Ética en Investigación. Resultados: la nube de palabras contextualizó y trajo subsidios para la identificación de dos categorías: comprensión de la necesidad de trabajo en equipo para planificar el alta hospitalaria y percepción de la necesidad de implementar herramientas como protocolos y flujos para orientar la preparación para el alta y la continuidad de la atención. Conclusión: los profesionales refieren que el alta hospitalaria se realiza según la rutina asistencial y sigue un modelo médico centrado.

O presente estudo objetivou analisar a perspectiva das gestantes sobre como ocorre o cuidado odontológico no contexto da Atenção Primária à Saúde. Trata-se de uma pesquisa de caráter qualitativo por meio de 16 entrevistas semiestruturadas com gestantes no segundo ou terceiro trimestre de gestação, em um município do interior paulista. Os dados foram submetidos à análise de conteúdo, modalidade temática. Observou-se que o acompanhamento odontológico nas Equipes de Saúde da Família e Unidades Básicas de Saúde têm foco na saúde geral da gestante e do feto, mas um olhar fragilizado para a saúde bucal. As gestantes apresentaram limitações relacionadas ao autocuidado bucal, bem como barreiras que interferiram no atendimento odontológico durante o período gestacional. Apesar da identificação da importância do atendimento odontológico no período gestacional, as gestantes enfrentam medos, receios, influências negativas de parentes, despreparo profissional e assistencial, que limitam a realização do pré-natal odontológico.


The present study aimed to analyze the perspective of pregnant women on how dental care occurs in the context of Primary Health Care. This is a qualitative study through 16 semi-structured interviews with pregnant women in the second or third trimester of pregnancy, in a city in the countryside of São Paulo. The data was subjected to content analysis, thematic modality. It was observed that dental care in the Family Health Teams and Basic Health Units focuses on the general health of the pregnant woman and the fetus but has a weak focus on oral health. Pregnant women presented limitations related to oral self-care, as well as barriers that interfered with dental care during the gestational period. Despite identifying the importance of dental care during pregnancy, pregnant women face fears, fears, negative influences from relatives, lack of professional and care preparation, which limit the provision of prenatal dental care.

Nasopharyngeal cancer (NPC) cases are often diagnosed in advanced stages. The complexity of clinical management for advanced-stage NPC requires thorough communication and shared decisions between medical professionals and allied teams. Incorporating a multidisciplinary team meeting (MDTM) for newly diagnosed NPC patients was chosen to facilitate collaboration and communication between physicians. This retrospective study aimed to compare the quality of care, clinical responses and survival between NPC patients treated with and without MDTM. Data on clinical responses, assessment visits, date of progression and death with progression-free survival (PFS), overall survival (OS), and hazard ratio (HR) were collected and analyzed with 95% confidence interval (CI) and significance set as P < .05. There were 87 of 178 NPC patients treated with MDTM. Revisions of diagnosis and stage occurred in 5.7% and 52.9% of cases during the MDTM. More clinical responses were achieved by patients treated with MDTM (69.0%vs 32.0%, P < .00). NPC patients who received MDTM treatment recommendation had a lower risk for progression (median PFS 59.89 months vs 12.68 months; HR 0.267, 95% CI: 0.17-0.40, P < .00) and mortality (median OS was not reached vs 13.44 months; HR 0.134, 95% CI: 0.08-0.24, P < .00) compared to patients without MDTM. Incorporating the MDTM approach into NPC management improves patients' clinical responses and survival.

OBJECTIVE: The objective of this study was to analyze whether patients undergoing esophageal atresia (EA) surgery benefit from a cross-disciplinary follow-up program, based on current clinical guidelines, implemented in our institution. MATERIALS AND METHODS: An observational, analytical, retrospective study of patients undergoing EA surgery from 2012 to 2022 was carried out. The results of a joint pediatric surgery and gastroenterology consultation program -which was implemented in 2018 and applies a protocol based on the new ESPGHAN-NASPGHAN guidelines- were analyzed. Patients were divided according to whether they had been treated before or after 2018. Quantitative variables -follow-up losses, anti-reflux treatment initiation and duration, and enteral nutrition initiation- and qualitative variables -prevalence of gastroesophageal reflux, anti-reflux surgery, respiratory infections, anastomotic stenosis, re-fistulizations, dysphagia, impaction episodes, need for gastrostomy, and endoscopic results- were compared. RESULTS: 38 patients were included. 63.2% had gastroesophageal reflux. 97.4% received anti-reflux treatment in the first year of life, with treatment being subsequently discontinued in 47.4%. Discontinuation time decreased by a mean of 24 months following program implementation (p< 0.05). A 4.6-fold increase in the frequency of pH-metries was noted following program implementation. The protocol standardized endoscopies in asymptomatic patients when they turn 5 and 10 years old. 25 endoscopies with biopsy were carried out after 2018, with histological disorders being detected in 28% of them. The number of follow-up losses significantly decreased following protocol implementation (p< 0.05). CONCLUSIONS: Digestive-surgical cross-disciplinary follow-up of EA patients has a positive impact on patient progression. Applying the guidelines helps optimize treatment and early diagnosis of complications.


OBJETIVOS: El objetivo de este estudio es analizar si los pacientes intervenidos de atresia de esófago (AE) se benefician de un programa de seguimiento multidisciplinar, basado en las guías clínicas actuales, implantado en nuestro centro. MATERIAL Y METODOS: Estudio retrospectivo, observacional y analítico incluyendo los pacientes intervenidos de AE entre 2012 y 2022. Se analizaron los resultados de la implantación en 2018 de un programa de consultas conjuntas de gastroenterología y cirugía pediátrica aplicando un protocolo basado en las nuevas guías ESPGHAN-NASPGHAN. Se dividieron a los pacientes tratados antes y después de 2018 y se compararon las variables cuantitativas: pérdidas de seguimiento, inicio y duración del tratamiento antirreflujo e inicio de nutrición enteral, y cualitativas: prevalencia de reflujo gastroesfoágico, realización de cirugía antirreflujo, infecciones respiratorias, estenosis de la anastomosis, refistulizaciones, disfagia, episodios de impactación, necesidad de gastrostomía y resultados de las endoscopias. RESULTADOS: Se incluyeron 38 pacientes. Un 63,2% presentaron reflujo gastroesofágico. El 97,4% tomaron tratamiento antirreflujo el primer año de vida que posteriormente se retiró en el 47,4%. El tiempo de retirada se redujo una media de 24 meses tras la aplicación del programa (p< 0,05). Se realizaron 4,6 veces más pHmetrías tras la implantación del programa. El protocolo estandarizó la realización de endoscopias en pacientes asintomáticos al cumplir 5 y 10 años. Se realizaron 25 endoscopias con tomas de biopsia después de 2018, detectando alteraciones histológicas en un 28%. El número de pérdidas de seguimiento se redujo de forma significativa tras la implantación del protocolo (p< 0,05). CONCLUSIONES: El seguimiento multidisciplinar digestivo-quirúrgico de los pacientes con AE genera un impacto positivo en su evolución. La aplicación de las guías facilita la optimización del tratamiento y el diagnóstico precoz de las complicaciones.
